Insurance, Documentation & Care Navigation

Organize care costs, coverage questions, and documentation without assuming that a diagnosis guarantees payment. Build a clear record and ask the right plan-specific questions.

Separate care decisions from coverage decisions

A clinically appropriate service may still have plan-specific coverage limits, and a covered service may not be the best clinical choice for you. Ask both the treating team and the insurer about their separate requirements.

This guide is general care-navigation education, not individualized legal advice or a guarantee of reimbursement. Rules differ by insurer, plan, location, and service.

Ask before booking or purchasing

Confirm whether the provider and facility are in network, whether a referral or prior authorization is needed, what documentation is required, and what out-of-pocket costs may remain.

For compression or equipment, ask about eligibility, prescriptions, quantities, replacement schedules, suppliers, and fitting requirements. A general lymphedema benefit should not be assumed to cover Lipedema without the relevant eligibility criteria.

Keep a focused record

Save the plan documents, benefit response, authorization information, written estimate, claim, explanation of benefits, and any denial. Record the date, person, reference number, and substance of important calls.

Ask clinicians to document accurate symptoms, functional limitations, diagnoses, and previous care. Do not exaggerate history, copy another person's medical-necessity letter, or select a diagnosis code yourself to obtain payment.

If a claim is denied

Read the stated reason and the actual deadline. Ask what information would address the issue and how to submit the required review or appeal. HealthCare.gov explains US internal appeals and external review; your plan's process and applicable rules must be checked.

Keep copies and proof of submission. A clinician, benefits adviser, or relevant consumer-assistance program may be able to help. Do not assume one universal appeal deadline applies to every situation.

Reduce the navigation burden

Choose the next concrete task: confirm a benefit, request a missing record, obtain a written estimate, or prepare questions for a clinician. A short checklist and a trusted helper can be more useful than trying to solve the entire system in one sitting.

Bring the practical barriers to your health coach. We can help organize your personal pathway, but the insurer, clinician, or appropriately qualified adviser must confirm coverage, coding, and legal requirements.
